Case Submission Page

To submit a pet for Aftercare with Peaceful Heart Aquamation, please fill out the appropriate form below. Upon submission, the pet’s information will go to our staff and we will pick the pet’s body up for aftercare. For private cremations, remains will be returned to you as soon as possible, typically within 7-10 business days. Please be sure to include any special instructions in the notes area as well as if we should expect to find any implants, if implants are found they are not typically returned unless requested, so please let us know if a client wants the implants returned.
